Each senior will get assigned with one student RN/ LPN case manager and a student volunteer , who will be overseen by RN . The Student case manger will identify the needs of the senior and will work with student volunteer , the community resources and the organization in getting the help they will need . We will be allocating maximum of 100 dollars/month /per senior towards their co-pay , medications reimbursement . When possible, we would be able to provide fresh food . The student volunteers will work with them to assist light house keeping , delivery of food , transportation as well help the senior to write their Memoir by each decade.
